The information listed below is all it takes to fill in the form. Most of the information involves personal details, travel documents, specifications of the stay, and the bio-data page of the passport along with a photograph for verification.
Moreover, you need to be clear about where you are going to stay and for how long so that you can fill in the accommodation details in the form.
Here are the requirements you need to fulfill:
- Fill in the application form and submit it 7 days before the date of arrival into Vietnam.
- Personal details including name, date of birth, and others.

- The polish passport along with the scanned copy of the bio-data page (Sample for passport’s bio page). Ensure that it will remain valid until after 6 months of date of arrival into the country and has 2 blank pages, at least, for stamping the visa.
- The accommodation details section might ask you for the name of the hotel and the city you are planning to lodge in and for how long.
Once you have filled in all the details, you can use your debit or credit card to make the payment for the fee.
